What is a Medicare Part D drug plan?

Medicare Part D is optional prescription drug coverage offered by private insurance companies approved by Medicare. It helps pay for prescription drugs and is available as a stand-alone Prescription Drug Plan (PDP) for people with Original Medicare, or as part of a Medicare Advantage plan that includes drug coverage (MAPD). Enrollment is time-sensitive — missing your window can result in a late-enrollment penalty that is permanent.

What is a Part D late-enrollment penalty?

If you go without creditable prescription drug coverage for 63 or more consecutive days after your Initial Enrollment Period ends, you may owe a late-enrollment penalty when you do enroll. The penalty is 1% of the national base beneficiary premium for each month you were without creditable coverage, added permanently to your monthly premium.

What does creditable prescription drug coverage mean?

Creditable coverage means your current prescription drug coverage is at least as good as Medicare Part D. Coverage from an employer, union, retiree plan, VA, or TRICARE may qualify. If your coverage is creditable, you can delay Part D enrollment without penalty — but you must have written proof from the plan provider.

Why does my pharmacy matter when choosing a Medicare drug plan?

Medicare drug plans contract with pharmacies at different pricing tiers — preferred and standard. A pharmacy can be in-network but not preferred, and that difference can significantly change what you pay. Before enrolling, check whether your specific pharmacy is preferred or standard in that plan's network using the Medicare Plan Finder at medicare.gov.

Should I review my Medicare drug plan every year?

Yes. Medicare drug plans can change their drug lists, tier placements, cost-sharing amounts, and pharmacy networks every year — even when your medications do not change. The Annual Enrollment Period runs October 15 through December 7. Reviewing your plan annually using your current medications and pharmacy is the most reliable way to avoid unexpected cost increases.

What should I do if my Medicare drug plan denies or restricts a medication?

Start by asking your pharmacy what message they are seeing — whether the drug is not covered, requires prior authorization, has quantity limits, or requires step therapy. Then call the plan to confirm the restriction. If the drug is covered but restricted, your prescriber can request a prior authorization or file an exception. Is the lowest premium Medicare drug plan the best choice?

Not necessarily. The lowest monthly premium does not always mean the lowest total yearly cost. What is the Medicare Part D $2,000 out-of-pocket cap?

Starting in 2025, the Inflation Reduction Act capped Medicare Part D out-of-pocket drug costs at $2,000 per year. Once you reach this cap, your plan pays 100% of covered drug costs for the rest of the year. This replaces the previous coverage gap (donut hole) structure and applies to all standard Part D plans and most Medicare Advantage plans with drug coverage. The cap applies only to drugs on the plan's formulary.
